Diabetes Self-Management Center

Recognizing community and regional healthcare needs, Carlsbad Medical Center opened the Diabetes Self-Management Center in July 2004. The Center works with patients of all ages who have been diagnosed with any type of diabetes.

Parents receive nutrition management along with education in all aspects of diabetes care. The Center supplies all clients with a free meter on their first visit. Our Diabetes Self-Management Center partners with your physician to help each patient attain good outcomes with their diabetes.  The Center accepts patients by physician referral only.

We welcome you visit the Center at 2402 W. Pierce Street, Suite 2A (located inside the CMC Outpatient Lab). You may telephone us at 575-887-4205 from 8 a.m. to 4:30 p.m. Monday through Friday.  We also see patients once a week (Tuesday) in Artesia at CMC's Multi-Specialty Clinic, located at 612 N. 13th Street, Suite C.

Clinical staff include Susan Dade, R.D., L.D., C.D.E., and Melissa Carter, R.N., C.D.E.  Both are Certified Insulin Pump Trainers.
